First Visit

smiling woman in dental chairYour initial appointment will consist of a consultation and evaluation explaining your diagnosis and treatment options. Occasionally, surgery may be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.

Please assist us by completing the following information at the time of your consultation:

  • Complete your paperwork online prior to your appointment  Patient Registration Online Forms. This will speed up your check-in process and reduce further contact and waiting in our offices. 
  • Bring in your surgical referral slip (handwritten or secure-emailed to our office) with any x-rays, if applicable.
  • A list of medications you are presently taking.
  • Your medical and/or dental insurance, bring the necessary completed forms/cards. This will save time and allow us to help you process claims on your behalf.

PLEASE NOTE: All patients under the age of 18 must be accompanied by a parent or guardian at their consultation visit.

A pre-operative consultation and physical examination is required for patients undergoing IV anesthesia for surgery. During your initial exam/consultation, our surgical team will review your medical history (i.e., diabetes, high blood pressure, artificial heart valves and joints, etc.) and current medications (i.e., heart medications, blood thinners such aspirin, Plavix, or Coumadin, osteoporosis medication such as Fosamax, Actonel, etc.) prior to any surgical procedure performed in our office.


If your dentist or physician has taken x-rays you may request for them to be forwarded to our office. If there is not enough time, please pick them up and bring them to our office.  If an x-ray or panorex is necessary, we will take one at any of our office locations.
