First Visit

smiling woman in dental chairYour initial appointment will consist of a consultation and evaluation explaining your diagnosis and treatment options. Occasionally, surgery may be performed on the same day as your consultation. However, a complex medical history and treatment plan will require an evaluation and a second appointment for treatment, on another day.

Please assist us by following these steps before your consultation:

  • Complete your paperwork online:  Patient Registration Online Forms to speed up your check-in process. 
  • Bring your surgical referral slip (handwritten or secure-emailed to our office) with any x-rays, if applicable
  • Have a list of medications you are presently taking
  • Bring your dental/medical insurance cards and forms, if applicable. This will allow us to process claims on your behalf

*Patients under 18 must be accompanied by a parent or legal guardian at their consultation visit.

A pre-operative consultation and physical examination are required for patients undergoing IV anesthesia for surgery in our office. During your initial exam/consultation, our surgical team will review your medical history (i.e., diabetes, high blood pressure, artificial heart valves, and joints, etc.) current medications (i.e., heart medications, blood thinners such aspirin, Plavix, or Coumadin, osteoporosis medication such as Fosamax, Actonel, etc.) prior to any surgical procedure performed by our doctors.

Radiographs-

If your dentist or physician has taken x-rays you may request for them to be forwarded to our office by secure-email. If there is not enough time, please pick them up and bring them to your appointment.  If an x-ray or panorex is necessary, we will take one at any of our office locations.
